A VMAT planning solution for neck cancer patients using the Pinnacle planning system

Abstract:
Purposes: Volumetric modulated arc therapy (VMAT) allows for rapid delivery of highly conformal dose distributions. The aim of this study is to evaluate VMAT planning using the Pinnacle planning system for neck cancer. This treatment technique, recently used for clinical applications, was proposed in 2008 by Karl Otto and this year is implemented, for the first time, in Romania.A set of computed tomography data were selected for this localization and used for delineation of planning target volumes (PTV) and organs at risk, and also for the calculation of dose distribution using the inverse planning method.The total prescription dose was 70 Gy (56 Gy in PTV1 and additional 14 Gy in PTV2) for neck cancer.By the aid of the Pinnacle planning system, acceptable treatment plans were achieved, the coverage of 95 % of the PTV being between 95 % and 99 % of the prescribed dose. The radiation dose delivered to organs at risk was under the required constraint dose. VMAT proved to be a safe, qualitative and advantageous modality treatment for these types of localizations. We succeeded to achieve a dosimetric quality with an adequate coverage of PTV a good homogeneity and conformity. By analyzing the radiation treatment plans, obtained with the Pinnacle planning system, it results that this treatment technique is several times faster for dose distribution calculation and for radiation dose delivery as compared to other conformal radiotherapy techniques.
