Title: A treatment planning study comparing VMAT with 3D Conformal
Radiotherapy for prostate cancer using a Pinnacle planning system

Abstract: In Romania, in the last 40 years, the most common method for malignant tumor treatment, with ionizing radiations, was cobalto-therapy, where it was used for radiation delivery, the 60Co sources. Since 2001, specialized linear accelerators have been acquired for this type of specialized treatment. Certainty, both the flexibility and accuracy of the beam, produced by the linac, had replaced the 60Co machine utilization. One of the newest techniques, in Europe, for cancer treatment is volumetric modulated arc therapy (VMAT), implemented for the first time, in 2008. In this study we are comparing the treatments plans for prostate cancer using 3D conformal radiotherapy (3D CRT) and VMAT technique. A comparison study for prostate cancer using external beam radiotherapy, to evaluate the dosimetric impact of these treatment methods was performed. We compared the dose distribution obtained for prostate cancer with3D CRT using 6 static treatment beams, and dual arc VMAT, optimized with SmartArc algorithm. A dose of 74 Gy was prescribed to prostate and minimum 54 Gy to seminal vesicles, in 37 fractions.The desired dose distribution for prostate tumors is more easily achieved by VMAT technique than by 3D CRT. Concerning the dose restrictions, the VMAT technique as compared to 3D CRT, provides a good avoidance of organs at risk, respecting the dose limits from RTOG 0415 trial. Also, the time of treatment delivery is shorter for VMAT technique.
